What Teero records that a payer-pattern review would draw on:
- Denials. Each denial is classified and corrected or appealed. Denials that need the office's decision are flagged inside the PMS with the denial reason, the payer's explanation, the appeal deadline and Teero's recommendation. That puts the payer and the reason on the claim, where a PMS report can group them.
- Delays. Claims over 30 days, meaning insurance A/R over 31 days, are followed up with the payer. Because posting runs daily, every business day, the PMS aging report reflects what has actually paid, so slow payers show up as slow rather than as unposted.
- Underpayments. Checked against the fee schedule before any write-off, so a plan that consistently pays under contract shows up as a pattern rather than disappearing into adjustments.
- Secondary claims. Sent when the primary pays, so a secondary balance is not mistaken for a primary delay.
